Tick the box when the dose is taken. Write the time next to it if you took it late.
This sheet is not a prescription. Follow your doctor's or pharmacist's instructions and the package leaflet.

How to use a medication log
Put the log next to where you take your medicines, with a pen tied to it. Tick the box straight after each dose – not later from memory. If a dose was late, write the time next to the tick; if it was skipped on purpose, write a short note why.
A filled-in log is useful at appointments: it shows your doctor how the treatment really went, including the doses that were missed. For short courses such as antibiotics, a 7- or 14-day sheet makes it obvious when the course is finished.
When a paper log isn't enough
Paper only works if you look at it. If doses get missed because nobody reminds you, a reminder app that keeps asking until the dose is confirmed solves the actual problem – and keeps the log for you.

Medication log questions
How long should the log be?
Choose 7 days for a short course, 14 days for a fortnight between visits or 28 days for a monthly overview. Print a new sheet when one is full.
Can caregivers use it?
Yes. Write the person's name at the top. If several people give the medicines, ask everyone to initial their ticks.
What if I missed a dose?
Leave the box empty and note it. What to do about a missed dose depends on the medicine – the package leaflet usually explains it, and your pharmacist can help. Don't take a double dose unless you're told to.
Is what I type saved anywhere?
No. The log is built from what you type and nothing is stored or added to the page address.
