How to make a medication list that actually helps
A good medication list answers four questions at a glance: what (the medicine and its strength), when (morning, midday, evening, bedtime), how many and how (with food, on an empty stomach, with water). Copy the details from the prescription label or the package leaflet, not from memory.
Keep one copy where you take your medicines and one in your wallet or bag. Bring it to every appointment and to the pharmacy, and update it the same day anything changes – including vitamins, supplements and medicines you buy without a prescription.
Plan or log?
This template is a plan: what to take and when. If you also want to tick off each dose, use the printable medication log – one box per dose and day.

Medication schedule questions
What should a medication list include?
The name and strength of each medicine, how many to take and when, how to take it (e.g. with food) and why you take it. Add over-the-counter medicines, vitamins and supplements, plus allergies, and the date you last updated it.
Can I save the schedule as a PDF?
Yes. Click "Print or save as PDF" and choose "Save as PDF" as the printer. On iPhone, use Print and then share the preview.
Is what I type saved anywhere?
No. The sheet is built from what you type and nothing is stored or added to the page address. Close the page and it's gone.
Does this replace my doctor's instructions?
No. Copy the schedule from your prescription or leaflet. If anything is unclear, ask your pharmacist or doctor.
